Blues Policies Agreed to at Registration

Below are the Policies every parent agreed to when they registered their child for the 2024-25 Season

Waiver of Liability

I , the parent or guardian of the above named player, hereby give my approval for his/her participation in any and all activities of this program during the 2024-2025 season. I understand that the participation in the sport of ice hockey will expose the registrant to various risks incidental to the sport. I agree to assume these risks and agree that in the event of injury, I will waive any and all claims against GFLYH, Inc., its organizers, supervisors and or officials of said league.

RETURNED CHECK FEE and FEE FOR MULTIPLE AUTO PAY FAILURES

I agree to pay any fees or charges incurred as a result of this registration. If payment is made by check, I agree to pay a $25 fee if the check is returned for any reason. AUTO PAY: I Agree to pay any fees for multiple AutoPay Failures. Fees will not exceed the returned Check Fee. I understand it is my responsibility to contact the League Treasurer or Registrar if I am having payment issues and work out a payment plan.

BLUES RULES AGREEMENT

LIVINGSTON BLUES RULES AGREEMENT: I and my child agree to abide by all rules developed by Geneseo Youth Hockey Organization, Western New York Amateur Hockey Association, Empire Amateur Hockey Conference, NYSAHA and USA Hockey. PLAYER / ASSOCIATION COMMITMENT: After this registration is completed by the Parent, no movement to another association will be allowed until the conclusion of the appropriate State or National Tournaments in March/April unless there are extenuating circumstances. A request to be released after this registration is completed and prior to the conclusion of the appropriate State or National Tournaments must be submitted to and approved by the appropriate Section President. I agree my player will be registered with and participate for the Geneseo Livingston Blues for the 2024-2025 season.

PHOTO PERMISSION

PHOTOS: From time to time, photos are taken of players playing hockey. Do you give permission for pictures of your child, with no name association, to be put on the public areas of our website?

Rink Behavior and Parent/Guardian Supervision of Children

Parents are responsible for supervising all of their children when they are in any rink. However there were numerous issues last season at the SUNY Geneseo Rink and other rinks we need parents to be aware of and agree to: a.) Players must exit the ice promptly when ice time ends and only enter the ice after the Zamboni has exited. b.) Coaches, Managers and Locker Room Monitors are responsible for supervising the players in the Locker Room by USA Hockey policy but parents are responsible for supervising their children at all times outside of the locker room and when not on the ice. So if players (or siblings) are in hallways before/during/after ice times or players/siblings are in the stands during practices or games - they must be supervised by their parents or guardian. c.) Only Players and Coaches for the team may be on the benches during practices and games. No one is permitted to be on the benches while the Zamboni is resurfacing the ice.
